GENERAL INFORMATION ENROLLMENT SHEET
- Enrollment is open to children who will be age 3, 4, 5, 6, 7 or 8 by September 30th.
- The enrolled child must be toilet trained prior to enrollment.
- Children are accepted irrespective of race, gender or religious affiliation.+
- To be eligible for enrollment, parent must present the following at the time of registration:
- Original Birth Certificate ++
- Record of current Physical Examination completed by a physician
- Up-to-date Immunization Records – Including TB Test
- Social Security Registration (number) ++
- Proof of Address
- Picture ID (for pick-up purposes)
++ Present Forms to Office Administrator for recording, not for copying.

*School calendar will follow that of the Norfolk Public School System.
*The A-Beka Curriculum will be used for Christian Education and Academic Subjects.
